By CTF you mean your Using a Custom theme as CXMB?
Well if you mean CFW, Probably some Protected Info (Mac Address, ID Storage, etc.) are corrupted and must be fixed. Im not the guy into this type of Problems since I don't have my PSP Phat Fixed. You can ask icejack1 or USMarine for these type of situations.
I've seen them reply to this type of Problems and seems that you need a Pandora Battery. So Get it Ready.

Well like SolidState said you can try using a pandora battery and mms to reinstall the firmware but instead of Gen, go for 5.00 M33-6. It should not freeze up unless you over tax the ram in the psp meaning that you either have a lot plugins on or use some kind of custom theme like CXMB and the reason that this doesn't happen on the 2000 is because the 2000 has more ram. Try disabling plugins and/ or reinstalling 5.00 M33-6.
Hope your psp works!

When it freezes, do you have to shut it down and reboot, or does it shut down by itself after a few seconds of freezing? The reason I ask this is because the Phat PSP's have a series of different Motherboards.
The TA-079, 081, 082, 086.
Motherboards TA-079 and TA-081: Pommel: 0x00000103
Motherboards TA-082 and TA-086: Pommel: 0x00000112
The Pommel is the part of the PSP's Motherboard Event logger and shuts down the PSP in the event of a crash.
Chances are your Motherboard is a 79 or a 81, which are the older Motherboards.
I am not sure if your Custom Firmware isn't fully compatible with your Motherboard, but a simple switch of Firmware can tell us. Try 5.00M33-6
Also, it can also be because of a faulty Flash. The Flash0 stores the PSP's firmware while the Flash1 stores the themes. If it only happened in the XMB, then chances are it was the Flash1, only. However, you mentioned that it also happens in games, so that's not the main issue, given it to be an issue at all. I would recommend that you make a back up of your current Flash and format it.
Now, this is the final advice I have. Try going into the Recovery Menu and set the CPU to the highest possible setting, both in the XMB and in Game. Also, make sure that Speed Up MS is set to ALWAYS.
Try these things and reply with the results.
